| | Mainly people just smudge / blur out the background, and leave the foreground objects untouched. At least that's how I've seen it done.

| | I read your question totally wrong and I do apologize. They way I add it, is by using the pen tool to mask out the object I want to keep untouched or sharpened. Add a Gaussian Blur to the background and then maybe sharpen the foreground objects. Just depends on what looks good.
